This product is a calcium channel blocker. It is used for the treatment of angina pectoris; hypertension (alone or in combination with other antihypertensive agents).

Dosage:

1. The dose of nifedipine should be adjusted gradually according to the patient's tolerance and control of angina pectoris. Overdose of nifedipine can lead to hypotension.

2. Start with a small dose. The usual starting dose is 10mg/dose, taken orally 3 times a day; the usual maintenance dose is 10-20mg/dose, taken orally 3 times a day. For some patients with significant coronary artery spasm, it can be used up to 20-30mg/dose, 3-4 times a day. The maximum dose should not exceed 120mg/day. If the condition is urgent, 10mg/dose may be taken chewed or sublingually and re-dosed according to the patient's response to the drug.

3. It usually takes 7-14 days to adjust the dose. If the patient has significant symptoms and the condition is urgent, the dose adjustment period may be shortened. Depending on the patient's response to the drug, the frequency of attacks and the dose of sublingual nitroglycerin, the dosage of nifedipine may be adjusted from 10-20mg to 30mg/dose 3 times a day within 3 days.

4. In hospitalised patients under strict monitoring, the dose may be increased by 10mg every 4-6 hours depending on the control of angina pectoris or ischaemic arrhythmia.

Adverse reactions:

1. Peripheral oedema (peripheral oedema is dose-related, with an incidence of 4% at 60mg/day and 12.5% at 120mg/day); dizziness; headache; nausea; malaise and facial flushing (10%) commonly occur after dosing. Transient hypotension (5%), most often without discontinuation (transient hypotension is dose dependent, occurring at 2% at doses <60mg/day compared to 5% at 120mg/day). Angina pectoris occurs in individual patients and may be associated with a hypotensive response. Palpitations; nasal congestion; chest tightness; shortness of breath; constipation; diarrhoea; gastrointestinal cramps; abdominal distention; skeletal muscle inflammation; joint stiffness; muscle spasms; nervousness; tremors; hypersensitivity; sleep disturbances; blurred vision; and balance disorders were also seen (2%). Syncope (0.5%), no longer occurs with dose reduction or in combination with other anti-anginal agents.

2. Rarely anemia; leucopenia; thrombocytopenia; purpura; allergic hepatitis; gingival hyperplasia; depression; paranoia; momentary blindness at peak blood levels; erythematous limb pain; antinuclear antibody positive arthritis, etc. (<0.5%).

3. Possible serious adverse reactions: incidence of myocardial infarction and congestive heart failure 4%; incidence of pulmonary oedema 2%; incidence of arrhythmia and conduction block each <0.5%.

4. Allergic hepatitis, rash and even exfoliative dermatitis may occur in hypersensitivity to this product.

Contraindications:
Contraindicated in patients with hypersensitivity to nifedipine.
